Become a Patient

Here at The Parkinson’s Plan, we have become skilled at determining who we are able to help. Once you submit an application, you will be required to book a 20-minute consultation with us. The consultation is not a medical intake. It is a quick zoom appointment to learn more about each other and determine if we are a good fit to work together. There is a $50 charge for the 20-minute consultation. If we decide to work together, this fee will go towards the fee for your initial program. If after the 20-minute consultation, both parties decide this is a good fit, you will complete our new patient intake forms, practice policies, waivers, and program deposit collection of $150.00. The deposit is completely refundable up to 24 business hours before your appointment. If you cancel within 24 hours of your scheduled appointment or do not show for your scheduled
appointment, the deposit is non-refundable. The $50.00 fee for the 20-minute consultation is non-refundable.
